The Dutch company LogoBorduurstudio have produced a YouTube video of a computer controlled embroidery machine creating a QR Code. The company based in Barneveld, The Netherlands will embroider a QR Code on either an item of clothing you send them or on any item from their range of casual and work wear. The QR Codes measure 6cm square without a border and 7cm square with a border. It costs $65 but I would guess that there is a significant amount of setup time involved. I had some excellent printed t-shirts made at Zazzle.com

Here is an excellent idea that coulde be used in many industries for entering information for a phone call.
Maricela Final’s QR Code idea is a quick and efficient time saver for credit union member’s and staff. Members point their smart phone at the QR code imprinted on the back of the plastic credit union issued ATM card, Debit Card, Pre-Paid card, Gift, and Visa/MasterCard credit card. The QR (Quick Response) codes on the back side of plastic cards will be embedded with the card information typically used when calling the 800 number to verify information pertaining to the card, such as balance inquiries, pending transactions, last 10 transactions on account, promotions, credit union rates, branch locations or nearby ATM’s, etc..
By offering a QR code on the back of all credit union issued cards, members can access their account information in seconds, without having to enter a member number or 16 digit card number. Additionally, a four digit pin number will be requested when scanning the QR code in order to access secured personal information.
